We believethat home health services are an essential part of the health delivery system.We understand  that by providing continuing care to clients in their own homes, home health services minimize the effects of illness, accident, or disability to the people we serve.

 

Health education is an integral part of our program.

We also recognize the intrinsic worth of aged, diseased and/or disabled persons.  Agile Home Health Services recognizes that these persons are entitled to retain or regain their maximum participation in society.

 

We acknowledge that the aging process is an unavoidable part of life. We understand chronic disease and disability may be a part of the process of aging.

In providing care to individuals and their families, we recognize that they have the right to participate in planning for their care, to make decisions about their health care, and to maintain their independence as long as feasible.

 

Agile Home Health Services recognizes that staff behavior, attitudes, knowledge, and understanding have an important influence on helping clients attain and maintain their optimum health.

In Summary, we are committed to the rehabilitation of aged, diseased, and/or disabled individuals within the structure of their home environments, allowing maximum contribution, independence, and retention of self-respect.

 

When rehabilitation or recovery is not realistic, we commit to providing dying individuals with appropriate care and coordinated services, supporting their dignity and self-respect.

¨ Monthly Infection Control Surveillance And Reporting

¨ Quarterly Monitoring Of Client Perception Of Care, Treatment And Expectations.

¨ Daily Monitor Of Adverse Events And Outcomes Of Process Or Services

¨ Monthly Diagnosis Monitor

¨ Client Safety  And Improvement Outcomes

 

OUR MANAGEMENT TEAM

Our team is lead by HealthCare Professional with many years of health care experience & a strong resolve to ensure that all clients receive the highest quality of healthcare & services.  Our clinical team is led by a Registered Nurse & a physical therapist with Many years of home healthcare, acute care, long term care, & ambulatory care experience

***CLINICAL STAFF SUPERVISON***

                 Clinical Supervisor initiates and implement all patient admissions  then,

                A.)  Educates and train the patient, family or nurse caregiver

                B.) Completes at least a monthly supervisory visit on all patients to include reassessment & evaluation of plan of care

                C.) Follow-Up on all family concerns

                D.) Revisit as needed to ensure quality of care .
